Oral bisphosphonates are listed as the nineteenth most commonly prescribed drugs according to over 17 million prescriptions in 2003. They are used for osteopenia/osteoporotic conditions which are commonly incited by inflammatory bowel disease or primary biliary cirrhosis, as a result of medications like steroids or simply post-menopausal aging. Bisphosphonates are potent inhibitors of osteoclastic activity in the normal bone turn-over process and thus reduces bone resorption. These drugs prevent, reduce and delay cancer-related skeletal complications in patients with multiple myeloma or metastatic malignant tumours. The more potent nitrogen-containing intravenous bisphosphonates are utilized for these dire conditions and this therapy is the one more largely associated with jaw bone necrosis following oral surgical procedures. Although rare, bisphosphonate-related osteonecrosis of the jaw (BRONJ) has also been reported in patients receiving the oral form of these drugs. Their utilization in the aging population for the prevention of osteoporotic fractures is growing and will become increasingly relevant among patients with existing dental implants as well as those who are implant candidates. This lecture will discuss the patho-physiology of BRONJ as well as the clinical manifestations, prevention strategies and existing treatment modalities for this potential surgical complication.
Dr. Zosky is a graduate of the University of Toronto and received his specialty training in Oral and Maxillofacial Surgery at Northwestern University. He is a Fellow of the International Association of Oral and Maxillofacial Surgeons and the Royal College of Dentists of Canada. He is a member of the International Congress of Oral Implantology and the American Academy of Implant Dentistry. Dr. Zosky is the author of several implant-related articles and a chapter in the W.B. Saunders textbook: “Endosseous Implants in the Maxillofacial Reconstruction”. He has lectured extensively in North America and abroad. He is past president of the Canadian Association of Oral and Maxillofacial Surgeons and past President of the Academy of Osseointegration Foundation. He has Fellowship status in the Academy of Osseointegration. Dr. Zosky is a consultant surgical staff at Trillium Health Sciences Centre. He is also an Associate in Dentistry, University of Toronto and teaches in the Implant Prosthodontic/Surgical Unit.
